Follow the steps below to submit your application for approval.
Payment is not collected in advance. You will be notified of the amount due once approved.

Business licenses cannot be handled by phone; a hard copy application is required.
If you prefer to apply for your business license in person, Lakewood's Administrative Services department staff will be happy to help.
A hard copy of the completed form is required to process the business license request.
Bring the completed form to the Administrative Services Counter, located nearest the Clark Avenue entrance to City Hall.
